Release of Liability Waiver

Please follow the link below to fill out  the new online waiver.

Please make sure you add your minor’s information by clicking the blue “ADD” button at the top (a new section will open up) and filling out that portion of the form.

Once you have filled out all required areas of the waiver and click the “submit” button, you should receive a confirmation number

(this means the waiver was successfully submitted to us).
